Why measure
- Patients stay with the plan when they can see the numbers move.
- GPs and specialists trust "LEFS 32 to 61" more than "much improved".
- Insurers and case managers ask for evidence that treatment is working.
- Over time, your own numbers show what works in your clinic.
A library of standard measures
Clinidiary includes a library of standard measures. Some of the most used:

| Pain | Numeric Pain Rating Scale (NPRS), Visual Analogue Scale (VAS) |
| Back and neck | Oswestry Disability Index, Neck Disability Index (NDI) |
| Lower limb | LEFS, KOOS, WOMAC, HOOS |
| Upper limb | SPADI, DASH |
| Tendons | VISA-A, VISA-P |
| Balance and falls | Berg Balance Scale, Timed Up and Go, FES-I |
Recorded on the case
Measures belong to a case, so a patient's knee and back scores never mix. Record the same measure on more than one visit and the trend appears. See treatment notes and cases.
Patients can answer at home
Send a questionnaire-style measure by email. The patient answers on their phone, Clinidiary scores it, and it joins the trend marked as answered by the patient. Clinic time goes on treatment. Intake forms work the same way; see intake forms and the patient portal.
In your letters and plans
Put the starting and latest scores in your letter to the GP, and in a WorkCover plan review. See GP and referrer letters and care plans and WorkCover paperwork.
A routine that works
- At the initial: a pain score and one function measure suited to the problem.
- Every four or five visits: repeat both.
- At discharge: repeat both and put them in the discharge summary.
